HOW TO REGISTER?

The patient by appointment

Appointment by coming to hospital directly	You should submit you treatment appointment form at the treatment area
Appointment by telephone or fax	You should bring along your ID & Insurance card to the reception desk.

The patient without appointment

First visit	Please submit your ID & Insurance card to the reception desk
Revisit	Please tell your name to reception desk

HOW TO PAY?

After seeing a doctor, you can pay at the reception desk and then you can get a receipt.
